The Blissful Pup LLC Service Contract and Hold Harmless Agreement is as follows:

I, the pet owner, understand and am in agreement with The Blissful Pup LLC's following policies and agreements:

Pets are accepted for grooming only under the following circumstances:

• The pet is fit and healthy. Grooming which takes place on an elderly or infirm pet will be at the owner's risk. Grooming may expose pre-existing health\skin conditions for which The Blissful Pup LLC cannot be held liable.

• The pet is flea and tick free. In the event that fleas are discovered on your pet during the groom, grooming services will be stopped and you will be asked to pick up your pet immediately. Ticks are removed for an additional fee.

• In the event of an emergency, in your absence, you authorize The Blissful Pup LLC to contact the nearest Veterinarian and authorize the Vet to treat the pet as necessary at your expense. 

• Ear plucking services are provided by request if recommended by your Veterinarian and so long as no pain or discomfort is caused to your pup.

• We do NOT offer anal gland expression, as this should be performed by a licensed veterinarian. 

• We do NOT offer deskunking treatment.

• “De-matting" or complete coat removal will dramatically alter your pet's appearance. This procedure may expose pre-existing health\skin problems for which The Blissful Pup LLC cannot be held liable. A dematting fee will be applied to the cost of grooming services.

• Proof of up-to-date Rabies and DHPP/DAPP are required for all services, non-core vaccinations such as Bordetella and Canine Influenza vaccines are highly recommended. Puppies must have completed their core vaccine series plus two weeks for full immunity for their health and safety prior to first groom.

• Final price of groom may vary due to services provided, condition of coat, frequency of grooming, length of groom, matting, fleas, ticks, behavior, health conditions, weight, stature and age. Pups may be weighed at their appointment. Time in excess of what is typical for a pup of their size and coat type will be charged additionally at a rate of $1/minute rounded up to 15 minute increments.

• Payment is to be made at the time of service. 

• To cancel or reschedule, please contact us at least 48 hours in advance. Fees may apply if less than 48 hours notice is given.

1. Current Vaccinations/Veterinarian Information: Owners agree to verify their pet(s) is current on core vaccines Rabies and DHPP/DAPP. Non-core vaccines such as Bordetella and Canine Influenza vaccines are highly recommended. Proof of Vaccination shall be provided, as well as current Veterinarian information prior to the grooming appointment. Special circumstances will be taken into consideration with Vet documentation.

2. Aggressive or Dangerous Pets: Owners MUST inform if their pet(s) bites, has bitten, or is aggressive to people, other pets or specific grooming procedures. Muzzles may be used if necessary. Muzzling will not harm your pet and protects both the pet and the groomer. The right is reserved to refuse/stop services for such pet(s) at any time before or during the grooming process.

3. Mat Removal: Pets with matted coats need extra attention during their grooming session. Mats left in a pet’s coat will only grow tighter, not allow for air circulation to the skin, and can strangle the pet’s skin, or eventually tear it open. Mats can be very difficult to remove and may require the pet to be shaved. When necessary, removing a heavily matted coat includes risks of nicks, cuts or abrasions due to warts, moles, skin folds and inflamed skin trapped in the mats. Heavy matting can also trap moisture and urine near the pet’s skin allowing hotspots, mold, fungus or bacteria to grow, causing skin irritations that existed prior to the grooming process. Fleas, ticks and other opportunistic parasites may be hiding under the mats to further irritate your pet's skin. Torn skin from mats can also harbor maggots. After effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions, ear bruising (hematomas), and failure of the hair to regrow. Shaved pets are also prone to sunburn and should either have sunscreen applied daily or should be kept out of the sun until the hair grows sufficiently to protect the skin. In some cases, pets can also exhibit brief behavioral changes.

Extra charges for matted pets may apply. Prevention is the best defense against matting, and The Blissful Pup LLC will be happy to show you how to care for your pet’s coat so that matting does not recur. Please ask about the best methods for maintaining your dog’s coat.

Understanding these risks, you authorize The Blissful Pup LLC to proceed with the dematting or matt removal process and hereby hold harmless and release The Blissful Pup LLC from any liability associated with the above-mentioned process and any and all medical problems that may be uncovered and/or occur during or after the stripping and/or dematting process by signing below. Should your pet need veterinary care after or during this process, you agree to pay any and all veterinarian fees. A dematting fee will apply as dematting causes excessive wear and tear and/or permanent damage to tools. The dematting fees can range from $40-$100.

4. Accidents: There is always the possibility an accident could occur. Grooming equipment is sharp. Even though extreme caution and care is used in all situations, possible problems could occur, including nail quicking, cuts, nicks and scratches. Contact must be made within 24 hours of your pup's grooming services with The Blissful Pup LLC with regards to possible grooming related concerns.

5. Parasites: If you suspect your pet has fleas or ticks, prompt and thorough action on your pet is needed. Flea infestation can lead to tapeworm and other health problems. If fleas or ticks are found during the grooming process, the groom will be stopped and you will be asked to promptly pick up your pet. Full groom service pricing will be charged in these instances. The pet can return to The Blissful Pup LLC for grooming services 7 days after starting a Veterinarian prescribed flea and tick medication (not over the counter). If ticks are found, we strongly suggest that you have your pet tested for Lyme Disease. Parasites are a health hazard to your pet, as well as to humans. Ticks will be removed at the cost of $10 per tick.

6. Senior Pet/Medical Release: I understand that my pet(s) may or do have a medical condition(s) and/or is/are/will be senior(s). I am aware that grooming can cause stress that can exaggerate existing problems or expose new problems, or even lead to a serious medical event or death during or after the groom. I understand there are mental and behavioral changes in senior dogs or dogs with medical conditions, and they are groomed for comfort and cleanliness in a style that will not add to their stress. I hold harmless and release The Blissful Pup LLC from any liability should any problem/medical issues occur. I am solely responsible for any and all medical bills related to my dog including transportation costs, and should an emergency arise, I give permission to The Blissful Pup LLC to seek veterinary care at the nearest veterinary clinic. I understand that The Blissful Pup LLC has the best interest of my pet in mind and will do everything to keep my pet safe. If The Blissful Pup LLC feels that grooming will be too stressful for my pet, the groom will be stopped immediately, and I will be contacted.

I give permission to The Blissful Pup LLC to obtain emergency veterinary treatment for my pet. I will not hold The Blissful Pup LLC responsible for any accident or injury of my pet during the grooming procedure.

7. Service Time and Pick Up Time: I understand that the timing and length of services may vary as working with dogs can be unpredictable. It is recommended to schedule grooming services when not pressed for time. This will allow for a stress free experience for all. Your flexibility is greatly appreciated by your groomer and your pup, although every effort will be made to stay on schedule. There are no boarding facilities or crates so please pick your dog up promptly. You will receive a call or text notifying you of the estimated pickup time of your dog. After 15 minutes of notified pick up time, late pick up will be charged at a rate of $1/minute rounded up to 15 minute increments.

8. Photos/Social Media: This release authorizes The Blissful Pup LLC to take photos and videos of your pet for client records and/or for marketing/website/social media pages.

9. Payment: Payments accepted via Credit Card, Zelle or cash. You understand that pricing is subject to change and due upon completion of services.

10. Satisfaction: The Blissful Pup LLC will always work hard to give your pet the full description of services and haircut that you want. However, please keep in mind that your pet's comfort and health are always the number one priority. Our approach is that of compassionate care, dog led luxury personalized grooming. Services are provided to the extent that is comfortable, safe and accepted by your pet(s). This is part of our personalized care model to ensure that grooming is a positive experience for your pet. You understand that the full price of services is due even if your pup is unable to receive all of the expected services due to their behavior or discomfort. If you are dissatisfied with the groom, please let The Blissful Pup LLC know. This will allow The Blissful Pup LLC to make any necessary corrections and take notes for the next grooming appointment. Any concerns about the grooming appointment must be requested within 24 hours after the grooming appointment.

11. Transportation: If any transportation of the listed pets is required or requested by you (or your agent) from The Blissful Pup LLC you agree to hold The Blissful Pup LLC harmless for any accident, damage, loss, injury or claim for the listed pet(s).

12. Dog Accessories: The owner of the dog, or the owner’s agent, is responsible for ensuring the dog accessory is suitable for their dog, that the product fits correctly and does not cause the dog any discomfort, and that it is safe for their dog to wear. Always supervise pets when they are wearing accessories. The Blissful Pup LLC accepts no legal responsibility for the use or misuse of dog accessories.

By signing this release in the field below, you are agreeing that you have read and reviewed this Service Agreement for accuracy, and that you understand the contents of this contract. You affirm that you are the rightful legal owner of the pet(s) for which services are being rendered. You authorize this signed contract to be valid approval for future grooming services, permitting The Blissful Pup LLC to accept telephone, sms, online, and/or email reservations for services without additional signed contracts or written authorization. By signing this agreement you (or your agent) agree to hold The Blissful Pup LLC harmless from any accidents, damage, injury, loss, or claim of/for the listed pet(s), including for known or unknown conditions, to The Blissful Pup LLC. It is also further understood and agreed that the terms of this agreement can change at any time, without notice, and will override any and all prior signed contracts or releases. It is further understood that this contract applies to all pets groomed by The Blissful Pup LLC.

The undersigned has read and understands the above contract and releases, and as the owner or agent of/for the pet(s) agrees not to hold The Blissful Pup LLC, it's owners, members and/or operators responsible for the injury, illness or death of their pet or for any expenses incurred because of injury, illness or death of any and all pets listed and groomed by The Blissful Pup LLC. I have read, understood and agreed to all of the terms and conditions above.

 

 

Pet Owners (or their Agent) that seek services from The Blissful Pup LLC are bound by The Blissful Pup LLC Service Contract and Hold Harmless Agreement whether or not a signature is obtained from the Pet Owner (or their Agent).
